Statement of consent to the use of speech and image
Permission (consent) to disseminate the image
In accordance with the provisions of Article 81(1) of the Act of 4 February 1994 on Copyright and Related Rights (Journal of Laws of 2021, item 1062) and Article 6(1)(a) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation (Official Journal of Laws of 2016). EU. L. 2016.119.1 as amended) I hereby allow the free dissemination of my statement and image along with identifying data (including name and surname) by the Organizers, i.e. AGH University of Krakow, Al. Mickiewicza 30, 30-059 Krakow, NIP: 6750001923, REGON: 000001577, www.agh.edu.pl (hereinafter referred to as AGH) and the TRY IT Foundation with its registered office in Krakow at Al. Mickiewicza 30, 30-059 Krakow, entered in the Register of Entrepreneurs of the National Court Register and in the Register of Associations, Other Social and Professional Organizations, Foundations and Independent Public Health Care Institutions of the National Court Register, No. KRS 0000859303, using NIP 6772456275, REGON 387052706 (hereinafter referred to as Try IT).
Dissemination of the image can take place in traditional and electronic form – in the form of photographs, film materials, audio and video recordings with the possibility of framing and composition for promotional, informational, cultural, educational purposes, related to the tasks carried out by AGH and Try IT.
The consent includes the dissemination of my image in the following ways:
- publishing on the AGH website at: https://agh.edu.pl/ and other websites in the agh.edu.pl domain set aside for the scientific and didactic activities of the AGH University of Krakow, the website of the Try IT Foundation at https://www.tryit.org.pl/ and partners cooperating with the Organizers;
- publishing on websites and social networks related to the Conference, including in particular on the https://pp-rai.pl
website; - posting in social media channels on AGH profiles, including in particular on the following social networks:
- https://facebook.com/AGH.Krakow,
- https://x.com/agh_krakow,
- https://www.youtube.com/@tvagh,
- https://instagram.com/agh.krakow,
- https://tiktok.com/@agh_krakow
- and on profiles set aside for the scientific and didactic activities of AGH University of Krakow or Try IT;
- all forms of audiovisual exploitation, including the online broadcast of the Conference
This consent to the processing of the image is non-exclusive and is granted without time and territorial restrictions regarding the dissemination of the image. In the event of withdrawal of this consent, the dissemination of the image made before the date of withdrawal remains in force and will not entail the need for AGH University of Krakow and Try IT to take actions aimed at removing the already disseminated image.
At the same time, I waive my rights related to the control and approval of each use of materials with my image.
Submission of this statement is a prerequisite for registration for the event.
Information from the personal data administrator
Pursuant to Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/WE (General Data Protection Regulation), hereinafter referred to as GDPR, We would like to inform you that:
- The joint administrators of your personal data are:
- AGH University of Krakow, al. A. Mickiewicza 30, 30-059 Cracow, NIP: 6750001923, REGON: 000001577, www.agh.edu.pl (hereinafter referred to as AGH),
- The TRY IT Foundation with its registered office at Al. Mickiewicza 30, 30-059 Kraków, entered into the Register of Entrepreneurs of the National Court Register and the Register of Associations, Other Social and Professional Organizations, Foundations and Independent Public Health Care Institutions of the National Court Register, KRS No. 0000859303, NIP 6772456275, REGON 387052706, www.tryit.org.pl. (further Try IT).

- Your personal data will be processed in order to promote the scientific and didactic activities of the AGH University of Krakow, Try IT and the PP-RAI Conference;
- Each of the Joint Administrators is responsible for:
processing of personal data in accordance with the principles set out in Article 5 of the GDPR;
- proper protection of personal data – in particular against access by unauthorized persons and unauthorized modification of data;
- exercising the rights of natural persons referred to in Articles 15-22 of the GDPR, taking into account possible restrictions resulting from the provisions of law;
- ensuring accountability in the implementation of obligations arising from the GDPR;
- keeping documentation describing the manner of personal data processing, including a register of data processing activities (requirement of Article 30 of the GDPR);
- reporting to the supervisory authority a violation referred to in Article 33 of the GDPR that occurred within its own structures;
- notifying a person of a data breach, if it may cause a high risk of infringement of the rights or freedoms of natural persons.
- Your personal data will be processed on the basis of Article 6(1)(a) of the GDPR, i.e. on the basis of your consent.
- We would like to inform you that at the time of posting your image on social networks, the data is transferred to a third country, and due to the form of publication, the data may be made available to an unlimited number of people around the world.
- Your personal data may be made available to entities that are authorized to do so under the provisions of law and to entities to which the Co-Administrator has entrusted the data in order to properly perform services for the purpose and scope necessary for these activities.
- Your personal data will be processed by the Joint Administrator until your consent to disseminate your image is withdrawn, and after its withdrawal for the time necessary to pursue or defend against claims. In the event of withdrawal of consent, the dissemination of the image made before the date of withdrawal remains in force and will not entail the need for AGH University of Krakow and Try IT to take steps to remove the already disseminated image.
- You have the right to request from the Joint Administrator access to your data and the right to rectify them, the right to delete data, the right to limit data processing, the right to object to processing in cases and under the conditions specified in the GDPR.
- You have the right to lodge a complaint with the President of the Office for Personal Data Protection if you believe that the processing of your personal data violates the provisions of the GDPR.
- Your personal data will not be processed in an automated manner, including profiling.
- Giving your consent to the processing of personal data is a requirement necessary to participate in the Conference, and the consequence of not giving it will be the inability to participate in the Conference.
